Change of Status Forms

In order to keep informed as to a current ProMatch member's status, staff request that ProMatch members complete and turn in the following forms, as needed:

Leave of Absence: If you need to leave ProMatch for a period of time (minimum 1 week, maximum 2 months) for any reason other than work, please complete the Leave of Absence form, so that we do not deduct service hours during your absence.

Request to Re-Activate: This form is for inactive members who want to reactivate their membership following a recent Leave of Absence or Return to Work. This form tells staff to add your name to the ProMatch active roster. Please also check to see if your profile is still active - if your profile has been deactivated, please create a new profile and submit it to ProMatch staff. (Note: If you have been inactive for over 60 days, please meet with staff to discuss your re-activation.)

Return to Work: Congratulations! You have found a job and are about to go back to work! Please complete the Return to Work form so that we can successfully close your file.

Completed forms can be sent to ProMatch staff either through the post office or via fax, or you can drop a completed form off at ProMatch. At the bottom of this page is our mailing address and fax number. You will not be able to email these forms to staff, as the information you enter in the web page form will not save.
